Our services Home from Hospital Are you 65 or over, a resident of the Borough of Kingston and/or registered with a Kingston GP? Are you due to be, or have you recently been, discharged from Kingston Hospital? Are you a carer of someone like this and worried about managing when they are back home? You may be entitled to our Home from Hospital service which aims to help you get home, safely and supported. Information for patients and carers We can help with... Supporting our clients and their informal carers to think through and prepare for all aspects of the return to home Follow-up support after discharge and introduction to other Staywell services e.g. Help at Home Handyperson Social Prescribing, Social activities to improve and maintain wellbeing, Information & Advice, including older age benefit checks and applications PLUS, up to 6 hours FREE practical support with the following: Preparing the home for discharge / change in functional ability (e.g. to remove or bring a bed downstairs / make space for a hospital bed) Awaiting equipment deliveries Shopping, prescriptions, light meal preparation Help with medical appointments Help arranging transport Staywell also offers a chargeable key safe installation service: (£95 to supply and fit a ‘police-approved’ Supra C500 key safe) Information for professional referrers Service criteria: Borough of Kingston upon Thames resident, and/or registered with Kingston GP, aged 65+, living alone or with another vulnerable person. We can carry out holistic assessment, supporting clients and their informal carers to prepare for the return to home. Our experienced Home from Hospital Coordinators can facilitate onward referral to PAC (Proactive Anticipatory Care) and social prescribing.
